My personal sites I design for 1024x768 as most people use it now days (and most new computers have it set as default as well). However, when I design for clients I have to design for 800x600 because its amazing people still use that resolution. I personally use 1680x1050 and its hard to imagine using 800x600 for daily use.
By designing around 800 width your make sure everybody will be able to view the site correctly. However, sometimes it is a pain to only have 800 width (even more if you plan to use 3 columns), it means your content column is pretty small. However, the best thing you could do is just use a fluid layout so it stretches to whatever the resolution is or browser size.
The choice is usually between 800x600 and 1024x768, or something that stretches. twofivethreetwo is right about the fluid layout.
Generally the 'safe option' is still to go with 800 width. But increasingly some mainstream sites are now designing for 1024 width. At a rough guess (and you should check this) there might still be 10% of people with ancient 800x600 width screens, but the numbers are falling every day. On the other hand, we are now increasingly seeing mobile devices (like the Nokia N80, for example) that have 800px wide screens, so that's a new development to keep an eye on.
In my view the *ideal* dimension is to have a screen that stretches between 800px and 1024px (but doesn't go on stretching forever). Like this one, for example. You need to use max-width and min-width plus a hack for IE6.

I normally don't allow my clients that much control over the design process (but my clients show me sites that they like that were made in 1999, so that might be why). With 14% still using 800x600 its a must to design around that if its for a client. I hate working with 800x600 but I have to for all my clients, I normally just do fluid layout though. However, depending on your sites content you could go with a different size. For example this site is mainly for designers (non of which should be using 800x600) so it would be safe to say that using 1024x768 would be safe and shouldn't turn away visitors. On my personal site I get maybe 2 visitors a month using 800x600 so I don't tailor to them.
If it's a graphical site I design to fixed width 780px or there abouts. Heavily text driven sites I'll use 80% liquid layout. Different tools for different jobs, innit.
